New Client Intake Forms

Two weeks prior to your child’s first session, your child’s therapist will send you new client intake forms to be completed online, signed electronically, and returned to Building Blocks prior to your child’s first session.  To protect your child’s privacy, we will provide you with a personalized password to access these password protected documents.  In the event you do not receive these forms in this time frame, please contact your child’s therapist and request that that forms be resent so they can be completed and reviewed prior to your child’s first session.

Case History Form

Approximately two weeks prior to your child’s first session, you will also receive a password protected case history form that should be completed and returned to your child’s therapist at least one week prior to your child’s first session.  Before you sit down to fill out the case history form, grab your child’s medical records from the pediatrician, his or her baby book, & a cup of coffee.  Filling out the case history form can be a time consuming process, but is very helpful to us as we prepare for your child’s first session.  You may not know the answers to some of the questions immediately.  The time you take to relay detailed & accurate information to us about your child’s development is very valuable in helping us get to know your child & tailor the therapeutic process to meet his or her needs.  Thank you for taking the time to provide us with this information.
